WHO IS ELIGIBLE TO ENROLL
You are eligible to participate in benefit plans if you are a “Regular” employee who regularly work. 30+ hours per week.
Your coverage begins the first of the month following 30 days of continuous employment.
You may enroll your eligible family member(s), including:
- Your legal spouse or domestic partner
- Your dependent child(ren), up to age 26 (regardless of student or marital status), including stepchild, legally adopted child, child placed with you for adoption, child of domestic partner, or a child for whom you or your spouse/domestic partner are the legal guardian of
- Your unmarried child(ren) age 26 or older who is/are mentally or physically disabled and who relies on you for full support and care. Certain documentation is required
ENROLLING AND MAKING CHANGES
You can make changes to your benefit elections during the year within 30 days of a qualifying life event, which include:
- Marriage, divorce, or legal separation
- Birth, adoption of a child, or placement for adoption of a child
- Loss or gain of other health coverage for you and/or dependents
- Change in employment status (i.e. change to part-time employment status)
- Moving out of the area
- Death of dependent
- Change in Medicaid/Medicare eligibility for you or a dependent
- Receipt of Qualified Medical Child Support Order
If such a change occurs, you must make the changes to your benefits within 30 days of the Qualifying event date. Documentation may be required to verify your change of status. Failure to request a change of status within 30 days of the qualifying event may result in your having to wait until the next open enrollment period to make your change; this includes the enrollment of a newborn child. Please contact Human Resources to make these changes.
WHEN COVERAGE ENDS
If your employment with Unrivaled Brands ends, your coverage and that of any enrolled dependent will end on the last day of the month in which eligible status is lost. Depending upon the circumstances, of your termination you may be able to continue coverage under COBRA.
Other circumstances which may result in termination of coverage for you and/or your dependents include:
- Change in employment status
- Dependent child turning age 26
